Bay Valley Foods Issues Allergy Alert on Undeclared Fish, Soy and Wheat in "America’s Choice® Classic Caesar Dressing"

FOR IMMEDIATE RELEASE -- Westchester, Ill.-- March 31, 2008 -- Bay Valley Foods is recalling 535 cases of "America's Choice® Classic Caesar Dressing" because some of the bottles produced in February 2008 have an incorrect ingredient label on the back of the bottle.

The incorrect label on the back includes ingredient and nutrition information for Chunky Blue Cheese Dressing and does not declare the presence of fish, soy and wheat. Consumers who have allergies to fish, soy and wheat run the risk of a serious or life threating allergic reaction if they consume the mislabeled product.

No illnesses have been reported to date in connection with this alert, and there is no risk to consumers who are not allergic to fish, soy and wheat.

The recalled bottles of "America's Choice Classic Caesar Dressing" were distributed through Great Atlantic & Pacific Tea Company (A&P) stores including A&P, Super Fresh, Food Basics USA, and Waldbaum's in Connecticut, New Jersey, Pennsylvania, Delaware, New York, and the District of Columbia.

The recalled product, "America's Choice Classic Caesar Dressing" comes in a 16 ounce plastic bottle and is marked with a "best before" date code of 02-09-09 that can be found on the back label. If the ingredient label on the back is for "Chunky Blue Cheese Dressing," consumers may return the product to the store where it was purchased for a full refund.

Stonyfield Farm Announces Nationwide Voluntary Recall of 6-Ounce Fat Free Blueberry Yogurts

FOR IMMEDIATE RELEASE -- Londonderry, NH -- March 28, 2008 -- Organic yogurt maker Stonyfield Farm has announced a voluntary product recall of its non-fat blueberry yogurt products.

The recall comes in response to the possibility of glass fragments in the product. Although the company believes the problem is not widespread, it is taking this precautionary measure to ensure the safety of its consumers.

Affected products would be 6-ounce Fat Free Blueberry yogurts carrying one of following product codes printed along the cup bottom that start with the following dates:

There are no reports of injury. People who bite into or swallow a fragment could possibly be injured, prompting this precautionary recall.

Stonyfield Farm is also advising its distribution network to remove the 6-ounce fat free blueberry yogurt from the shelves immediately. The product is sold at natural food stores and major grocery retailers nationwide.

Consumers are instructed to return opened and unopened containers to their retailers. Those returning product will be reimbursed for the full value of their purchase.

FDA Warns Consumers about "Total Body Formula" and "Total Body Mega Formula"

Distributor recalls dietary supplement products after reports of adverse reactions The U.S. Food and Drug Administration is advising consumers not to purchase or consume Total Body Formula in the flavors of Tropical Orange and Peach Nectar, or Total Body Mega Formula in the Orange/Tangerine flavor. The liquid dietary supplement products may cause severe adverse reactions, including significant hair loss, muscle cramps, diarrhea, joint pain and fatigue.

The Total Body Formula products are sold in eight-ounce and 32-ounce plastic bottles. The Total Body Mega Formula is sold in 32-ounce plastic bottles. Both products are distributed by Total Body Essential Nutrition of Atlanta. The company is the sole distributor of the products and has voluntarily recalled Total Body Formula in the flavors of Tropical Orange and Peach Nectar and Total Body Mega Formula in Orange/Tangerine flavor.

The Florida Department of Health recently provided reports to the FDA on 23 individuals who experienced serious reactions to these products seven to 10 days after ingestion. In all cases, the reactions included significant hair loss, muscle cramps, diarrhea, joint pain and fatigue. The FDA subsequently learned and is investigating a report that some individuals in Tennessee using the same products have experienced similar reactions.

FDA laboratories are analyzing samples of the products to identify the cause of the reactions, including the possibility that the products contain excessive amounts of selenium, which is known to cause symptoms such as those described in the adverse events reported to the agency.


Selenium
, a trace mineral, is needed only in small amounts for good health.

The products have been distributed in Alabama, California, Florida, Georgia, Kentucky, Louisiana, Michigan, Missouri, New Jersey, North Carolina, Ohio, Pennsylvania, Tennessee, Texas and Virginia.

The FDA is advising consumers in all states to avoid using the products immediately and to discard the products by placing them in a trash receptacle outside of the home.

Consumers who have been taking the products and have experienced adverse reactions should consult their health care professional. Consumers and health care professionals can also report adverse events to the FDA's MedWatch program at 800-FDA-1088 or online.

One grocery store, Price Less "Deli Department", in Cave Junction Oregon received cut product and all have been contacted under our food safety recall policy.

No illnesses have been reported to date, however individuals should call their healthcare professional if any of these symptoms occur.

The products being recalled include: Simply Fresh Fruit Brand food service Fruit Mix in Syrup products dated “sell by 4 – 18 08” or earlier, food service Cantaloupe Chunks in Syrup products dated “sell by 4 – 08 8” or earlier, and retail and club store Simply Fresh, Fresh Cut Fruit Brand containing cantaloupe dated “sell by 3 – 29 8”or earlier. Retail and club store products involved were removed from sale prior to Monday, March 24. Foodservice distributors who sell their own brand, have been notified, and are recalling the products involved. Castleberry's Expects Restart

According to a top executive with Castleberry's Food Co., he is confident that the Augusta plant will restart sometime this week.

March 10, the plant on 15th Street was shut down, when the U. S. Food and Drug Administration and U. S. Department of Agriculture found deviations in how some of the equipment was being operated.

Senior Vice President Dave Melbourne stated, "we continue to work closely with the FDA and the USDA to reinstate our operating permit at our Augusta plant and hope to resume production early this week based on our current discussions with the agencies."

According to Mr. Melbourne the latest shutdown does not involve health or safety issues.

For two months beginning in July, the plant was shut down because botulism toxin was found in cans of chili sauce. The Centers for Disease Control reported that eight people were sickened in Texas, Ohio and Indiana.

The recall involved more than 90 of the plant's products and $35 million.

Currently there are two lawsuits pending against the company by people who said they were sickened.

During a February 27 inspection of the plant, deviations in some equipment operations revealed that undercooking of the meat could have resulted. Because of underprocessing caused the botulism outbreak, federal agencies decided to revoke the permit.

Over the past two weeks, most of the facilities over 330 employees have not worked.

Mr. Melbourne stated that he has been very grateful of the patience and understanding of the employees and customers during this time.

There are at least 49,000 that have been found to be eligible for the settlement, and more than 46,000 have sent in at least some of the required paperwork.

388 Cases of Illnesses Reported from Indoor Water Park Reported

After an outbreak of norovirus at an indoor water park in upstate New York, the number of people reporting illnesses is approaching 400.

According to state health officials 388 cases have been reported by people who visited the Six Flags Great Escape Lodge and Indoor Water Park earlier this month.

Two people were hospitalized and since have been released according to the health officials.

Norovirus is characterized by acute gastrointestinal pain, vomiting and diarrhea lasting up to two days. The illness is not considered to be dangerous to the general public.

So far, four of the families infected have filed a lawsuit against the water park, which is located about 50 miles north of Albany.

Salmonellosis Outbreak Traced to Imported Cantaloupe

Oregon health officials are urging consumers to discard certain brands of fresh cantaloupe in the wake of a salmonellosis outbreak investigation.

A major supplier of cantaloupe to the U. S. and Canada is Agropecuaria Montelibano. The product is grown in the Honduras and packaged by Angropecuria Montelibano, also known as Agrolibano.

In the middle of January, a rare strain of the disease, Salmonella Litchfield, affected consumers in the U. S. and Canada. More than 50 cases in 16 states have been confirmed, including five in Oregon and nine in Washington.

According to Dr. Willian E. Keene, an epidemiologist at the Oregon Public Health Division, "if you have any of this kind of cantaloupe at home, we recommend that you throw it out. If your cantaloupe has a sticker that says Guatemala or some other country, then it is not associated with this outbreak. If you can't tell where it came from, or if it came from Honduras, you could check with the store to see if it might be from this company."

Agrolibano distributes under many different label names.

"It is confusing, but unfortunately there isn't any magic want we can wave to tell from which field," he said. "If you are concerned about fruit that you have at home, just toss it."

The U. S. Federal Drug Administration (FDA) is working with packers and growers to pinpoint the source of contamination, but investigation may take months.

In the state of Oregon only five cases are confirmed. There have been 25 or more or have become ill. One, at least, has recovered from the illness.

In the U. S., over the years, cantaloupe and other melons have been the source of many outbreaks. It is just important to remember that once you cut into the melons, eat it right away or stick it in a sealed container in the refrigerator. Otherwise, bacteria will start multiplying on the flesh of the melon if left out.

Salmonella typically causes diarrhea, abdominal cramps, fever and usually lasts less than than week. Some are at increased risk for severe illness, particularly the elderly, infants or those with auto-immunodeficiencies.

FDA Warns of Salmonella Risk with Cantaloupes from Agropecuaria Montelibano

The agency detains products from the Honduran manufacturer The U.S. Food and Drug Administration has issued an import alert regarding entry of cantaloupe from Agropecuaria Montelibano, a Honduran grower and packer, because, based on current information, fruit from this company appears to be associated with a Salmonella Litchfield outbreak in the United States and Canada. The import alert advises FDA field offices that all cantaloupes shipped to the United States by this company are to be detained.

In addition, the FDA has contacted importers about this action and is advising U.S. grocers, food service operators, and produce processors to remove from their stock any cantaloupes from this company. The FDA also advises consumers who have recently bought cantaloupes to check with the place of purchase to determine if the fruit came from this specific grower and packer. If so, consumers should throw away the cantaloupes.

The FDA recommends that consumers take the following steps to reduce the risk of contracting Salmonella or other foodborne illnesses from cantaloupes:

Purchase cantaloupes that are not bruised or damaged. If buying fresh-cut cantaloupe, be sure it is refrigerated or surrounded by ice.

After purchase, refrigerate cantaloupes promptly.

Wash hands with hot, soapy water before and after handling fresh cantaloupes.

Scrub whole cantaloupes by using a clean produce brush and cool tap water immediately before eating. Don't use soap or detergents.

Use clean cutting surfaces and utensils when cutting cantaloupes. Wash cutting boards, countertops, dishes, and utensils with hot water and soap between the preparation of raw meat, poultry, or seafood and the preparation of cantaloupe.

If there happens to be a bruised or damaged area on a cantaloupe, cut away those parts before eating it.

Leftover cut cantaloupe should be discarded if left at room temperature for more than two hours.

Use a cooler with ice or use ice gel packs when transporting or storing cantaloupes outdoors.

The vials were manufactured by Baxter and then placed by AHP into individually labeled bags for use in pharmacy automation equipment. The AHP packages where sold to five hospitals in Georgia and California, all of whom were notified of the recall earlier this month. Baxter Healthcare will reimburse AHP for the recalled product.

and rod) NDC # 00641-0410-25, lot numbers 070095B, 070095C, 068286, 067755. AHP instructed customers to return any and all of these product lots remaining in inventory.

This recall was initiated due to the Baxter Healthcare’s recall which stated, "…voluntary recall of Heparin Sodium Injection to include all lots of single and multi-dose vial products, due to an increase in reports of adverse patient reactions including abdominal pain, burning sensation, chest pain, diarrhea, dizziness, drug ineffectiveness, dyspepsia, dyspnia, erythema, flushing, headache, hyperhidrosis, hypoesthesia, hypotension, including profound and refractory hypotension, increased lacrimation, loss of consciousness, malaise, nausea, pallor, palpitations, paresthesia, paresthesia (oral), pharyngeal adema, restlessness, vomiting/retching, stomach discomfort, tachycardia, thirst, trismus, and unresponsiveness to stimuli. The reports of profound and refractory hypotension usually occur with the first few minutes of bolus administration."
